I understand that my child will be participating in a personal soccer training session(s) provided by 1conic Academy, which may include physical conditioning, soccer drills, and other athletic activities. These sessions are designed to improve skill, performance, and physical fitness. *

Medical Authorization: I affirm that my child is in good physical condition and does not suffer from any condition that would limit participation in the training sessions. I agree to notify the trainer of any changes to my child’s health that could affect participation. In the event of an emergency and if I cannot be reached, I authorize 1conic Academy to seek emergency medical treatment for my child and agree to be responsible for any related costs. *

Assumption of Risk: I acknowledge that participation in a soccer tournament involves inherent risks, including but not limited to: physical contact, collisions with other players, falls, sprains, fractures, concussions, and other serious injuries. I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of the organizers or others, and I take full responsibility for my participation. *

Waiver and Release of Liability: In consideration of my child’s participation, I hereby release, waive, and discharge 1conic Academy, its trainers, coaches, assistants, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child during training sessions, whether caused by negligence or otherwise. *

Media Release: I grant permission for my child to be photographed or video recorded during training sessions. I agree that any such media may be used by 1conic Academy for promotional purposes, including social media and marketing materials. *

Refund Policy: Clients must provide at least 12 hours’ notice to cancel or reschedule a private session. Cancelled sessions made within 12 hours of scheduled session will be NON-REFUNDABLE and NON-TRANSFERABLE. No-Shows will also be charged the full session fee. *

Indemnification: I agree to indemnify and hold harmless 1conic Academy from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of my child’s involvement in personal training sessions. *
